Planning
Develop an overarching goal to address your prioritized health issue in the community.
Develop one short-term (within 6 months) SMART objective to address your prioritized health issue in the community for the population at large.
What interventions would you include in the plan of care as the community health nurse to address your short-term objectives? Include a Primary, Secondary and Tertiary intervention for the chosen objective for the general population. (Support the selection of your interventions with references)
How would you modify each of your short-term interventions to accommodate a vulnerable population to achieve equity? (Support the selection of your interventions with references)

APA Expectations
The written portion of the project must include APA 7th edition citations and referencing. Students should include: Cover page, Introduction, written sections covering Diagnosis, Planning, Implementation & Evaluation, a Conclusion and Reference page for the paper.
Video Presentation
For the group presentation prepare a 5-10 minute long video and discuss the following:
• Why you picked the community.
• One surprising thing about your community.
• What health issues are prominent in your community?
• Which health concern you chose and why.
• What interventions did you chose to address the health concern for vulnerable populations?
*The presentation should not be a recap of every detail you included in the written portion of the assessment. Please focus on the above criteria in the presentation. Photos or video of the community should be used to provide context to the criteria within this section. Please be sure there are no faces in the pictures or videos. Every member must contribute and present the group project. A list of references used in the presentation (this may be different than the paper) using APA 7th edition must be included at the end of the video presentation. The Video presentation must include each member on camera during their portion. Each member should be professional in appearance, word choice, grammar, tone, and be aware of non-verbal cues. One member of the group must submit the presentation into DB 5 and in the Dropbox for BB. It is the group’s responsibility to be sure that the submission plays once uploaded.
